BACKGROUND: Bariatric surgery is the most effective option to reduce weight in morbid obesity patients. The techniques most employed are the restrictive surgery laparoscopic sleeve gastrectomy (LSG), surgical procedures of intestinal malabsorption, and both types (restrictive and intestinal malabsorption) such as the Roux-en-Y laparoscopic gastric bypass (RYLGB). AIMS: To determine if LSG is more effective than RYLGB for weight loss. METHODS: A systematic review and meta-analysis was carried out, including five clinical trials and sixteen cohorts comparing LSG versus RYLGB in weight loss and secondary outcomes: resolution of comorbidities, postoperative complications, operative time, hospital stay, and improvement in quality of life. RESULTS: Excess weight loss was 10.2% (mean difference [MD] 10.2; 95%CI -10.14; -9.90) higher in patients undergoing LSG than in patients submitted to RYLGB. Diabetes mellitus type 2 was resolved in 17% (relative risk [RR] 0.83; 95%CI 0.77-0.90) of cases, more significantly after LSG, arterial hypertension in 23% (RR 0.77; 95%CI 0.69-0.84), and dyslipidemia in 17% (RR 0.83; 95%CI 0.77-0.90). Postoperative complications were 73% higher in patients undergoing RYLGB (MD 0.73; 95%CI 0.63-0.83). The operative time was 35.76 minutes shorter in the LSG (MD -35.76; 95%CI -37.28; -34.24). Finally, the quality of life improved more in patients operated by LSG (MD 0.37; 95%CI -0.48; -0.26). CONCLUSIONS: The study demonstrated that LSG could be more effective than RYLGB in reducing the percentage of excess weight, comorbidities, postoperative complications, operative time, hospital stay, and in improving quality of life.

Introduction: Neonatal mortality remains a critical concern, particularly in developing countries. The advent of machine learning offers a promising avenue for predicting the survival of at-risk neonates. Further research is required to effectively deploy this approach within distinct clinical contexts. Objective: This study aimed to assess the applicability of machine learning models in predicting neonatal mortality, drawing from maternal and clinical characteristics of pregnant women within an intensive care unit (ICU). Methods: Conducted as an observational cross-sectional study, the research enrolled pregnant women receiving care in a level III national hospital's ICU in Peru. Detailed data encompassing maternal diagnosis, maternal characteristics, obstetric characteristics, and newborn outcomes (survival or demise) were meticulously collected. Employing machine learning, predictive models were developed for neonatal mortality. Estimations of beta coefficients in the training dataset informed the model application to the validation dataset. Results: A cohort of 280 pregnant women in the ICU were included in this study. The Gradient Boosting approach was selected following rigorous experimentation with diverse model types due to its superior F1-score, ROC curve performance, computational efficiency, and learning rate. The final model incorporated variables deemed pertinent to its efficacy, including gestational age, eclampsia, kidney infection, maternal age, previous placenta complications accompanied by hemorrhage, severe preeclampsia, number of prenatal checkups, and history of miscarriages. By incorporating optimized hyperparameter values, the model exhibited an impressive area under the curve (AUC) of 0.98 (95 % CI: 0.95-1), along with a sensitivity of 0.98 (95 % CI: 0.94-1) and specificity of 0.98 (95 % CI: 0.93-1). Conclusion: The findings underscore the utility of machine learning models, specifically Gradient Boosting, in foreseeing neonatal mortality among pregnant women admitted to the ICU, even when confronted with maternal morbidities. This insight can enhance clinical decision-making and ultimately reduce neonatal mortality rates.

Objective: To compare the effects of high-intensity interval training (HIIT) and moderate-intensity continuous training (MICT) on adults with overweight and obesity. Outcomes, including changes in insulin sensitivity, weight, body mass index (BMI), waist circumference, and body fat, were analyzed. Methods: A systematic literature review was conducted. This review is registered in the International Prospective Register of Systematic Reviews (PROSPERO) under the number CRD42021281899. Clinical trials involving individuals who are overweight and obese and comparing HIIT with MICT effects on insulin sensitivity, weight, BMI, body fat percentage, and waist circumference were included. PubMed, Web of Science, Embase, and Scopus databases were searched using controlled vocabulary and free-text terms related to HIIT, obesity, and overweight. The search included studies published until September 2022. The Rob2 tool was used to assess the risk of bias. The results were presented through meta-analyses that provided summary estimators and confidence intervals. Subgroup analyses were conducted to assess the effect of the risk of bias on the outcomes. This research did not receive any specific funding. Results: Of the 2534 articles, 30 met the eligibility criteria. The intervention duration ranged from 4 to 16 weeks. The observed effects for each outcome were as follows: insulin sensitivity (p = 0.02), weight (p = 0.58), BMI (p = 0.53), waist circumference (p = 0.87), body fat percentage (p = 0.07), body fat mass in kilograms (p = 0.39). The level of evidence obtained was moderate except for waist circumference, which was rated as low. Limitations included heterogeneity in training protocols, measurements, and study duration. Additionally, a risk of bias was identified in these studies. Conclusion: HIIT and MICT did not significantly differ in their effects on weight, BMI, waist circumference, or body fat mass in adults with overweight and obesity. However, a moderate beneficial effect of HIIT was observed on insulin sensitivity. Therefore, further evidence is required to confirm these findings.

Anti-VEGF drugs like ranibizumab can be used to treat retinopathy of prematurity (ROP) by halting the formation of abnormal blood vessels, or lasers can be used to burn the edges of the retina where these vessels are present. The objective is to compare the efficacy for ROP between ranibizumab and laser therapy. Material and methods: Electronic searches will be carried out in medical databases with key words and controlled vocabulary terms. Randomized controlled trials (RCT) will be assessed. The primary outcome will be the full ROP regression. Two reviewers will extract the data using predefined forms and, to assess the quality of the study, we will use RoB 2.0, the tool for randomized controlled trials developed by the Cochrane Collaboration. We used a combination of the inverse-variance approach and random-effects models for the meta-analysis. Results: The eyes of 182 preterm infants who had ranibizumab treatment were assessed in a total of 364 eyes, and 135 infants received laser therapy. The follow-up period was between 6 and 24 months. Ranibizumab was not associated with greater regression of ROP compared to laser therapy in preterm infants (RR: 1.09, CI 95%: 0.95-1.24; p: 0.22). Also, ranibizumab was not associated with recurrence of ROP compared to laser therapy in preterm infants (RR: 3.77, CI 95%: 0.55-25.81; p: 0.22). Conclusions: The efficacy of ranibizumab compared to laser is very uncertain in terms of ROP regression and decreased ROP recurrence in preterm infants. Since the spread of the first cases of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection much progress has been made in understanding the disease process. However, we are still facing the complications of coronavirus disease 19 (COVID-19). Multiple sequelae may appear as a consequence of acute infection. This set of entities called post-COVID-19 syndrome involves a wide variety of new, recurrent or persistent symptoms grouped together as a consequence of the acute disease process. One of those that has attracted the most attention is the liver and bile duct involvement called post-COVID-19 cholangiopathy. This is characterized by elevation of liver markers such as alkaline phosphatase, bilirubin and transaminases as well as alterations in the bile ducts in imaging studies. Thus, a narrative review of the cases reported until the end of 2021 was carried out. From the findings found, we concluded that patients who have had COVID-19 or during the process have required hospitalization should remain under follow-up for at least 6 months by a multidisciplinary team.

<b><br>Introduction:</b> Acute appendicitis is one of the most frequent emergencies in hospitals around the world; it requires early surgical treatment in complicated cases. One of the challenges that the surgeon faces during appendectomy is when the base of the appendix is compromised by either a perforation or gangrene. To show the surgical strategies that have been reported, as well as the complications associated with a compromised appendicular base in a complicated acute appendicitis.</br> <b><br>Methods:</b> A bibliographic search was carried out in the databases of Pubmed, Embase, Web of Science, and Google Scholar. The search expression ("appendiceal stump closure" OR "Closure of the appendiceal stump" OR "Management of appendiceal stump") was used to search for articles. The inclusion criteria were observational studies (case reports, case series, or cross-sectional, case-control, or cohort studies).</br> <b><br>Discussion:</b> Different techniques have been reported for the treatment of a compromised appendicular base. Among the most used are primary closure, partial resection of the cecum, cecostomy tube, ileocecectomy, and right hemicolectomy. The most frequent complications are surgical site infection, intra-abdominal abscess, postoperative ileus, intestinal obstruction, and others.</br> <b><br>Conclusions:</b> The appendicular base, compromised by necrosis or perforation, requires adequate treatment in order to prevent dehiscence of the appendicular stump sutures and fecal peritonitis. A number of surgical options have been reported.</br>.

RESUMO RACIONAL: A cirurgia bariátrica é a opção mais eficaz para reduzir o peso em pacientes com obesidade mórbida. Três técnicas são mais empregadas: gastrectomia vertical laparoscópica (LSG), procedimentos cirúrgicos de má absorção intestinal e ambos os tipos, má absorção intestinal e restritiva, e bypass laparoscópico em Y-Roux (RYLGB). OBJETIVOS: Determinar a eficácia da LSG versus RYLGB na perda de peso. MÉTODOS: Uma revisão sistemática e meta-análise foi realizada incluindo cinco ensaios clínicos e dezesseis coortes comparando LSG versus RYLGB na perda de peso e resultados secundários, resolução de comorbidades, complicações pós-operatórias, tempo operatório, permanência hospitalar e melhora da qualidade de vida. RESULTADOS: A perda de excesso de peso foi 10,2% (diferença média (MD) 10,2; IC95% -10,14; -9,90) maior em pacientes submetidos a LSG do que em pacientes operados com RYLGB. Diabetes mellitus tipo 2 foi resolvido em 17% (risco relativo (RR) 0,83; IC95% 0,77-0,90), hipertensão arterial em 23% (RR 0,77; IC95% 0,69-0,84), dislipidemia em 17% (RR 0,83; IC95% 0,77-0,90), mais significativos após LSG. As complicações pós-operatórias foram 73% maiores em pacientes submetidos a RYLGB (MD 0,73; IC95% 0,63-0,83). O tempo operatório foi 35,76 minutos menor na LSG (MD -35,76; IC95% -37,28; -34,24). Finalmente, a qualidade de vida melhorou mais no LSG (MD 0,37; IC95% -0,48; -0,26). CONCLUSÕES: LSG é muito eficaz na redução do excesso de peso, comorbidades, complicações pós-operatórias, tempo e melhora na qualidade de vida.

OBJECTIVE.: To compare the survival of patients with chronic kidney disease (CKD) on hemodialysis (HD) versus peritoneal dialysis (PD). MATERIALS AND METHODS.: Survival analysis of a retrospective cohort of patients ≥ 18 years who started HD versus PD at the Victor Lazarte Echegaray Hospital from 2015 to 2019. We analyzed the following covariates: age, sex, diabetes mellitus as cause of CKD, temporary central venous catheter (CVC) as initial vascular access and glomerular filtration rate. Survival was calculated with Kaplan-Meier curves for the overall cohort and for age ≥ 60 years, diabetes mellitus as a cause of CKD and CVC. The risk of death was estimated by Hazard Ratio (HR) according to the Cox proportional hazards model for each covariate adjusted for dialysis type in a bivariate and multivariate analysis considering significant difference if the p-value < 0.05. RESULTS.: We included 368 patients on HD of whom 129 (35.1%) died, and 172 patients on PD of whom 66 (38.4%) died (p=0.455). The cumulative probability of survival at 60 months for HD was 30% and for PD was 37% with similar survival curves (p=0.719). The median survival time for HD was 32 months (IQR: 20-53) and for PD was 32.5 months (IQR: 18-57) (p=0.999). The covariates associated with higher mortality adjusted for dialysis type were age ≥60 years (HR 1.77; p<0.001) and diabetes mellitus as a cause of CKD (HR 1.63; p=0.002). CONCLUSIONS.: Survival of patients with CKD on HD and PD was similar.

Introduction: Early onset neonatal sepsis (EONS), particularly in preterm sepsis, is a potentially fatal issue. Evaluation of mean platelet volume (MPV) as an EONS predictor was the goal. Methods: Four databases were used to conduct a systematic evaluation of cohort and case−control studies. Up till the end of October 2022, 137 articles were found utilizing the search method. Following the review, 12 studies were included. Leukocytes, MPV, platelets, gender, birth weight, gestational age, mortality, and C-reactive protein (CRP) were all taken into account while analyzing the prediction of EONS. Inverse-variance methodology and the random-effects model were used. Using GRADE, the evidence's quality was evaluated. Results: Neonatal patients with sepsis had significantly higher MPV levels than do neonates without sepsis (MD 1.26; 95% CI 0.89−1.63; p < 0.001). An increased MPV during the first 24 h postpartum was associated with high CRP values and high risk of neonatal mortality. In the investigations, the MPV cutoff for sepsis patients was 9.95 (SD 0.843). Overall certainty of the evidence was very low. Conclusions: The increased MPV during the first 24 h postpartum may be predictive of EONS and mortality. Future studies are warranted.

OBJECTIVE: Demonstrate that simplified ERICVA (Valladolid Critical Limb Ischaemia Risk Scale) is useful in predicting amputation in chronic limb-threatening ischemia (CLI) after one year of revascularization. METHODS: A retrospective cohort study was performed. We analyzed the medical records of 93 patients over the age of 35 with the diagnosis of CLI who were treated in the Department of Internal Medicine, Orthopedics or in the Cardiovascular Surgery Unit of the Víctor Lazarte Echegaray Hospital and the High Complexity Virgen de La Puerta Hospital during the period 2015-2018. The simplified ERICVA score was determined in patients before surgical and endovascular revascularization. We included 31 patients who scored 2 or more points in the exposed group and 62 patients who scored less than 2 points in the group not exposed to amputation risk. The collected data was analyzed with the statistical program SPSS where the Relative Risk and significance was obtained with Pearson's Chi-square. The multivariate analysis was also carried out in order to obtain the adjusted relative risk. RESULTS: It was identified that the simplified ERICVA score greater than or equal to 2 points was more frequent in those who underwent amputation (90.3%) compared to patients who did not undergo amputation (4.8%), increasing the risk of amputation in those patients with CLI who underwent revascularization (RR: 18.67, P<0.001). It was also possible to identify that within the group of patients that showed a high risk of amputation according to the ERICVA scale, they had a higher risk of major amputation (RR: 9.32, P<0.001) as opposed to the risk of minor amputation (RR: 1, 89, P=0.193). Among the items of the simplified ERICVA scale, the preoperative neutrophil-lymphocyte ratio and hematocrit were significantly higher in the group of amputated patients (P<0.001). In addition, it was possible to identify that the score greater than or equal to 2 was independently associated with the risk of amputation in patients revascularized with CLI (RR: 13.5, P<0.001). CONCLUSION: In our patient population, the simplified ERICVA scale is useful in predicting major and minor amputation in critical limb ischemia after revascularization. The present data showed that the patients who had a simplified ERICVA score greater than or equal to 2 had a higher risk of major amputation compared to the risk of minor amputation. However, it is important to highlight that the impact on the prediction of minor amputation is greater because in some circumstances major amputation can appear as a complication of CLI.

Results. We included 368 patients on HD of whom 129 (35.1%) died, and 172 patients on PD of whom 66 (38.4%) died (p=0.455). The cumulative probability of survival at 60 months for HD was 30% and for PD was 37% with similar survival curves (p=0.719). The median survival time for HD was 32 months (IQR: 20-53) and for PD was 32.5 months (IQR: 18-57) (p=0.999). The covariates associated with higher mortality adjusted for dialysis type were age ≥60 years (HR 1.77; p<0.001) and diabetes mellitus as a cause of CKD (HR 1.63; p=0.002). Conclusions. Survival of patients with CKD on HD and PD was similar.

BACKGROUND: In neonates with meconium aspiration syndrome (MAS), continuous positive airway pressure (CPAP) may be more beneficial compared to endotracheal intubation (ETI). We evaluated the efficacy of CPAP in neonates with MAS. METHODS: Four engines were used to search randomized clinical trials (RCTs). We used relative risk (RR) and mean difference (MD) with 95% confidence intervals (95%CI) to assess the effect on dichotomous and continuous outcomes, respectively. In addition, we used the Paule-Mandel (PM) random effects model due to the anticipated lack of events. RESULTS: Three RCTs were included (n = 432). No significant difference was found in mortality (RR = 0.82; 95%CI = 0.54-1.25; I2 = 71%; p = 0.36), need for ventilation (RR = 0.49; 95%CI = 0.15-1.56; I2 = 71%; p = 0.57), and incidence of pneumothorax (RR = 1.24; 95%CI = 0.30-5.12; I2 = 0%; p = 0.77) in the CPAP group compared to the ETI group. Regarding secondary outcomes, compared to the ETI group, no significant differences were found in APGAR at one minute (MD = -1.01; 95%CI -2.97 to 0.94; I2 = 98%; p = 0.31), APGAR at 5 min (MD = -1.00; 95%CI = -2.96 to 0.95; I2 = 99%; p = 0.32), days of hospitalization (MD = -0.52; 95%CI = -1.46 to 0.42; I2 = 94%; p = 0.28), and cord pH (MD = 0.003; 95%CI = -0.01 to 0.02; I2 = 0%; p = 0.79). CONCLUSIONS: In patients with MAS, there is no significant effect of CPAP use compared to ETI on primary, specifically on mortality, need for ventilation, the incidence of pneumothorax, and secondary outcomes.

Introducción: La apendicitis aguda es una de las causas más comunes de abdomen agudo quirúrgico y dado que sigue siendo un reto diagnóstico, dispone de diversas escalas diagnósticas. Objetivo: Determinar la superioridad del nuevo score resultante frente al score de Alvarado en el diagnóstico de apendicitis aguda. Métodos: Se realizó un estudio de pruebas diagnósticas con el uso de la base de datos del Servicio de Cirugía del Hospital Regional Docente de Trujillo, entre febrero y diciembre del año 2015. Con los datos extraídos se elaboró un nuevo score diagnóstico que fue evaluado con el informe histopatológico y luego comparado con el score de Alvarado para evaluar su aplicación. Se calculó la sensibilidad, especificidad y los valores predictivos de ambos scores. Resultados: Se incluyeron en el estudio 312 pacientes, 177 fueron hombres (56,73 por ciento) y 135 mujeres (43,27 por ciento), con edad media de 31 años (±11,3). Con un total de 267 (85,58 por ciento) pacientes con apendicitis aguda confirmada por estudio histopatológico. El nuevo score clínico resultante de una regresión logística según la prueba de Wald, estuvo compuesto por 4 variables: sexo masculino, vómitos, automedicación previa y signo de Blumberg. Los resultados del área bajo la curva para el nuevo score clínico y el score de Alvarado fueron 0,711 y 0,707, respectivamente. Conclusiones: El nuevo score clínico es superior al score de Alvarado según el área bajo la curva, pero no en un valor significativo(AU)


Introduction: Acute appendicitis is one of the most common causes of acute surgical abdomen. Since it remains a diagnostic challenge, it has several diagnostic scales available. Objective: To determine the superiority of a new resulting score versus the Alvarado score in the diagnosis of acute appendicitis. Methods: A study of diagnostic tests was carried out using the database of the surgery service of the Regional Teaching Hospital of Trujillo, between February and December 2015. With the data collected, a new diagnostic score was elaborated and assessed with the histopathological report; and then, to assess its application, it was compared with the Alvarado score. Sensitivity, specificity and predictive values of both scores were calculated. Results: 312 patients were included in the study, 177 were male (56.73 percent) and 135 were female (43.27 percent), with a mean age of 31 years (±11.3). A total of 267 (85.58 percent) patients had acute appendicitis confirmed by histopathological study. The new clinical score resulting from a logistic regression according to the Wald test was made up of four variables: male sex, vomiting, previous self-medication and Blumberg's sign. The results of the area under curve for the new clinical score and the Alvarado score were 0.711 and 0.707, respectively. Conclusions: The new clinical score is better than the Alvarado score, according to the area under curve, but not by a significant value(AU)

La enfermedad COVID-19 se caracteriza principalmente por manifestaciones clínicas respiratorias, que pueden ser leves hasta muy severas, sin embargo, hay un grupo de pacientes que pueden cursar con eventos tromboembólicos en cualquier parte del cuerpo. Se realizó una búsqueda de información científica en tres bases de datos PubMed, Scopus y Web of Science, con el objetivo de describir y analizar las potenciales causas de la trombosis mesentérica asociada a la infección por SARS-CoV-2, así como los resultados clínicos, de los pacientes que presentaron y fueron tratados por trombosis mesentérica durante el curso de la enfermedad. Se han reportado diferentes mecanismos fisiopatológicos de eventos tromboembólicos asociados a la COVID-19, dentro de ellos se mencionan el estado de hipercoagulabilidad, una mayor producción de factor Von Willebrand, la expresión de la enzima convertidora de angiotensina 2 en los enterocitos del intestino delgado, que como respuesta a la infección pueden liberar mediadores inflamatorios y el estado de shock presente en las dos terceras partes de los pacientes críticos. Los pacientes con la COVID-19 y sobre todo aquellos que cursan con estadios graves pueden tener diferentes mecanismos que confluyen o exacerban un estado de hipercoagulación, que puede puede afectar cualquier parte del cuerpo como los vasos mesentéricos y llevar a una isquemia gastrointestinal que comprometa su viabilidad y termine en una resección intestinal por necrosis(AU)


COVID-19 disease is mainly characterized by respiratory clinical manifestations, which can be light to very severe; however, there is a group of patients who can present with thromboembolic events in any part of the body. A search of scientific information in three databases, PubMed, Scopus and Web of Science, was carried out with the aim of describing and analyzing the potential causes of mesenteric thrombosis associated with SARS-CoV-2 infection, as well as the clinical outcomes of patients who presented and were treated for mesenteric thrombosis during the course of the disease. Different pathophysiological mechanisms of thromboembolic events associated with COVID-19 have been reported, among them the hyper-coagulable state, an increased production of Von Willebrand factor, the expression of angiotensin-converting enzyme 2 in small intestinal enterocytes, which in response to infection can release inflammatory mediators, and the state of shock present in two thirds of critically ill patients. Patients with COVID-19 and especially those with severe stages may have different mechanisms that converge or exacerbate a state of hyper-coagulation, which can affect any part of the body such as the mesenteric vessels and lead to gastrointestinal ischemia that compromises its viability and ends in intestinal resection due to necrosis(AU)

OBJECTIVE: The objective of this study was to investigate whether glycated hemoglobin (HbA1c) is a valid surrogate for evaluating the effectiveness of antihyperglycemic drugs in diabetes mellitus (DM) trials. METHODS: We conducted a systematic review of placebo-controlled randomized clinical trials (RCTs) evaluating the effect of a treatment on HbA1c (mean difference between groups) and clinical outcomes (relative risk of mortality, myocardial infarction, stroke, heart failure, and/or kidney injury) in patients with DM. Then, we investigated the association between treatment effects on HbA1c and clinical outcomes using regression analysis at the trial level. Lastly, we interpreted the correlation coefficients (R) using the cut-off points suggested by the Institute for Quality and Efficiency in Healthcare (IQWiG). HbA1c was considered a valid surrogate if it demonstrated a strong association: lower limit of the 95 percent confidence interval (95 percent CI) of R greater than or equal to .85. RESULTS: Nineteen RCTs were identified. All studies included adults with type 2 DM. None of the associations evaluated was strong enough to validate HbA1c as a surrogate for any clinical outcome: mortality (R = .34; 95 percent CI -.14 to .69), myocardial infarction (R = .20; -.30 to .61), heart failure (R = .08; -.40 to .53), kidney injury (R = -.04; -.52 to .47), and stroke (R = .81; .54 to .93). CONCLUSIONS: The evidence from multiple placebo-controlled RCTs does not support the use of HbA1c as a surrogate to measure the effectiveness of antihyperglycemic drugs in DM studies.

Introducción: El riesgo de prematuridad abarca una amplia variedad de complicaciones al nacer, como enfermedades infecciosas, hemorragia intraventricular, deficiencias neurosensoriales, insuficiencia respiratoria y afectación de otros sistemas, así como problemas de crecimiento y desarrollo. Por tanto, la consecución del Objetivo de Desarrollo del Milenio de reducir la mortalidad infantil, depende en gran medida de la reducción de la mortalidad relacionada con el nacimiento prematuro, lo que lo convierte en uno de los desafíos más importantes para la salud pública moderna. Objetivo: Identificar los factores maternos asociados a prematuridad. Material y Métodos: Se realizó un estudio de tipo caso-control retrospectivo en 2000 recién nacidos vivos de un hospital público de Trujillo, entre el 2015-2019. Se dividieron los grupos según el criterio de edad gestacional. Se analizaron las variables clínicas maternas pareadas según prematuridad o a término, y se realizó un análisis bivariado y multivariado por regresión logística ajustada por confusores. Resultados: Se demostró que el número de controles prenatales menor a cuatro durante la gestación, está asociado a prematuridad (OR 2,65; IC95%: 2,21-3,18). Por otro lado, la ausencia de una infección del tracto urinario (OR 0,73; IC95%: 0,56-0,95), está asociado a menor riesgo de prematuridad. Conclusiones: El número de controles prenatales es un factor asociado importante para parto prematuro, mientras que la ausencia de una ITU puede estar asociada a menor tasa de partos prematuros en gestantes de riesgo.

RESUMEN La pandemia de la COVID-19 ha provocado aproximadamente 182 millones de casos y cerca de cuatro millones de muertes en todo el mundo, comprometiendo con mayor frecuencia a personas de 25 y 64 años de edad. Esta infección afecta principalmente al sistema respiratorio, pero otros órganos y sistemas pueden verse afectados a corto plazo como el neurológico. Una gran población de pacientes ha logrado superar la infección por el SARS-CoV-2, sin embargo, han comenzado a reportarse una serie de manifestaciones clínicas tardías, básicamente neuropsiquiátricas, dentro de ellas se ha referido a la ansiedad, depresión, deterioro del sueño, dolor muscular, mareos, cefaleas, fatiga, anosmia, entre otros; a este conjunto de manifestaciones tardías se le ha denominado síndrome neurológico post-Covid-19 y requiere por un lado, la atención de la comunidad médica para investigar las manifestaciones tardías o secuelas de esta enfermedad y por otro lado, una vigilancia médica ante la consulta de pacientes con estas manifestaciones.


ABSTRACT The COVID-19 pandemic has caused approximately 182 million cases and nearly four million deaths worldwide, most frequently involving people between 25 and 64 years of age. This infection primarily affects the respiratory system, but other organs and systems can be affected in the short term, such as the neurological system. A large population of patients has managed to overcome the SARS-CoV-2 infection, however, a serie of late clinical manifestations have begun to be reported, basically neuropsychiatric, including anxiety, depression, disorder of sleep, muscle pain, dizziness, headaches, fatigue, anosmia, among others; this set of late manifestations has been called post-Covid-19 neurological syndrome and requires, on the one hand, the attention of the medical community to investigate the late manifestations or sequel of this disease and, on the other hand, medical vigilance when consulting patients with these manifestations.

RESUMEN Diversas terapias son empleadas para el manejo del enfisema subcutáneo en adultos, pero no se ha encontrado alguna referencia del drenaje subcutáneo mediante la técnica de Beck en pacientes pediátricos. Presentamos el caso clínico de una paciente de catorce años de edad con un trauma cerrado debido a una caída desde una altura de diez metros. Al ingreso, la paciente muestra pérdida de conciencia y desarrolla un síndrome de distrés respiratorio, por lo que se le indica ventilación mecánica. Siete días después se produce un enfisema subcutáneo y se realiza una inserción con angiocatéter fenestrado empleando la técnica de Beck. El enfisema en la zona superior del tórax y el rostro disminuyó a las 24 horas, y a las 72 horas la resolución fue completa.


ABSTRACT Several therapies are used for the management of subcutaneous emphysema in adults, but no reference to subcutaneous drainage using Beck's technique has been found in pediatric patients. This is the clinical case of a 14-year-old female patient who presented a blunt force trauma that resulted from a ten-meter-high fall. On admission, she was unconscious and developed respiratory distress syndrome, thus mechanical ventilation was indicated. Seven days later, subcutaneous emphysema occurred, and a fenestrated angiocatheter was inserted using Beck's method. At 24 hours, the emphysema was reduced in the upper chest and face, and, at 72 hours, complete resolution was observed.

Objetivo: Realizar una revisión sistemática y metaanálisis para comparar terapias target más quimioterapia con quimioterapia en el tratamiento de pacientes con cáncer de mama triple negativo EC IV o recurrente. Material y Métodos: Una búsqueda sistemática de estudios clínicos aleatorizados para el tratamiento de cáncer de mama triple negativo en EC IV o recurrente. Los desenlaces de interés fueron Sobrevida Global (SG) y sobrevida libre de progresión (SLP). Se registró datos de cada estudio clínico para los desenlaces y se estimó RR. Resultados: 10 estudios fueron incluidos y los participantes oscilaron entre 53 a 902 por estudio. La sobrevida global observamos que la terapia target más quimioterapia en las pacientes con cáncer de mama triple negativo EC IV o recurrente estuvo asociado a una disminución del 15% del riesgo de mortalidad con un RR (IC 95%): 0,85 (0,70 a 1,03). En relación al nivel de riesgo libre de progresión observamos que en la intervención se presentó una disminución del 16% en el riesgo de progresión con un RR (IC 95%): 0,84 (0,74 a 0,95). La sobrevida libre de progresión en las pacientes con la intervención oscilo entre 2,8 a 9,7 meses, mientras que en la terapia convencional osciló entre 1,5 a 6,2 meses. Conclusión: Las pacientes con cáncer de mama EC IV o recurrente triple negativo que recibieron terapia target más quimioterapia presentaron tendencia a la disminución en el riesgo de progresión de enfermedad (SLP) y mortalidad (OS), sin embargo, ­­­­se requieren más estudios clínicos para validar su significancia.


Background:Breast cancer is the most common malignancy in women worldwide. Patients with triple negative breast cancer represent a subgroup with unfavorable prognosis especially in advanced stages of the disease. Several first-line therapeutic agents have been described. Objective: To perform a systematic review and meta-analysis to compare target therapies plus chemotherapy with chemotherapy in the treatment of patients with triple negative breast cancer Stage IV or recurrent. Asystematic search identified randomized Material and Methods: clinical studies for the treatment of triple negative breast cancer in Stage IV or recurrent. The outcomes of interest were overall survival (OS) and progression-free survival (PFS). Data from each clinical study were recorded for the outcomes and the relative risk was estimated. Results:10 studies were included and participants ranged from 53 to 902 per study. Regarding overall survival, we observed that target therapy plus chemotherapy in patients with triple-negative Stage IV or recurrent breast cancer was associated with a 15% decrease in the risk of mortality with a RR (95% CI): 0.85 (0.70 to 1.03). In relation to the level of progression-free risk, we observed that the intervention showed a 16% decrease in the risk of progression with a RR (95% CI): 0.84 (0.74 to 0.95). Progression-free survival in patients with the intervention ranged from 2.8 to 9.7 months, while in conventional therapy it ranged from 1.5 to 6.2 months. Conclusion:Patients with Stage IV or recurrent triple negative breast cancer who received target therapy plus chemotherapy showed a trend towards decreased risk of disease progression (PFS) and mortality (OS), however, more clinical studies are required to validate its significance.
